Staffing has been a problem for this trust for many years and this is being addressed through a major recruitment drive. During our inspection we saw several examples of good care and all of the people that we spoke with told us that they had been very happy with the treatment that they had received. The needs of patients living with dementia or having learning disabilities were considered and addressed. However the number of complaints has levelled off to below expected numbers in recent months. However one "Just a Minute" (JAM) - feedback cards available for people to complete - for the postnatal ward stated, "Postnatal care on readmission has been good. People on the post natal ward gave us the following feedback about the environment, "I like having a single room". Some patients told us they were aware of how busy ward staff were and this discouraged them from using their call bells unless absolutely necessary. However, we did find evidence that people�s needs were not always being met in a way that was safe and effective. Improvements included more effective leadership and awareness among staff of the need to protect patient’s privacy and dignity. We also raised concerns about the potential safety of people who were treated in the ED having initially been assessed in the UCC. The length of time from referral to treatment for surgery was now generally in line with, or better than, the national average. Comments included, "Quality of the care given by all the staff. Staff had access to safeguarding information and had a good awareness of this area of patient safety; however, safeguarding of vulnerable adult training was not always up to date across the various departments. One patient said �Staff have been reassuring and as good as you would hope them to be.� Another patient said �They are marvellous but work very hard. Staff had access to training and opportunities to gain competencies related to their area of work. As a busy Trust with a wide range of hospital and community-based health services, in addition to our team of permanent staff, Croydon Health Services also draws on a talented pool of health care professionals to work on a temporary basis.
In the emergency department patients could not always access the service when they needed it and the trust was not always meeting the national standards for waiting times. Maintenance on the medical care wards and equipment was not always undertaken in a timely way. The urogynaecology and pelvic floor reconstruction unit at Croydon Healthcare had an international profile in relation to research, provided courses to the obstetric community and had won many awards. They also told us that staff members were friendly and helpful. Patients and their families felt involved and listened to but medical patients said there was a lack of information related to their treatment. Patients had noticed this and it was reflected in feedback we got, although it is clear the trust has a history and reputation that is making it hard for it to move forward. A public interest disclosure letter had been received by the Care Quality Commission (CQC) prior to the visit taking place. Individuals generally felt that staff worked very hard however they made comments about staff being "overstretched", "hard pressed" and "overworked". We would like to thank all of the staff and people who helped to facilitate this review and share their experiences. However, medicine fridge temperature checks were not done regularly in some surgical areas. As the regulator, CQC considered these concerns and other information received about the service since our last review. We found concerns about the quality and safety of care in three of the four services we inspected. Within expected range with a value of 56%. In some cases where performance fell below the England average there was an improvement seen since the previous audit. Individual patient feedback on the wards that we visited was, in the main, positive. We also carried out unannounced inspections of areas where we thought there was a risk of poor care. The trust employs approximately 3,640 staff and has a team of 300 volunteers supporting the services. We attend a range of events throughout the year. Ensure that the planned improvements to parent accommodation in children's services is completed on time. More can be done to ensure the Critical Care Unit is delivering consistently safe and effective care. There was too much reliance on non-permanent staff outside of core hours and the unit was cramped. This information, along with feedback from people who used the service and information from Croydon Healthwatch, and local Clinical Commissioning Groups, helped us decide where to look during our inspection.
